Registered Nurse (RN) | Case Manager Location: West Hollywood, CA Agency: Revolution Healthcare Services Pay: $2,529 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/16/2026 About the Position Job Opportunity – Case Manager/RN – California Start Date: February 16, 2026 Location: West Hollywood, California Compensation: Details provided to qualified applicants Schedule: 40-hour work week with weekend coverage required; no on-call required; float within scope to meet facility needs Highlights: Work with a daily census of 255 patients with a 15:1 case management ratio Epic charting system utilized throughout the facility Serve diverse patient populations including adolescents, adults, and geriatrics Comprehensive interdisciplinary support including social services and transportation coordination 16 hours of orientation provided to ensure successful onboarding No time off restrictions after the first two weeks of assignment Requirements: Active California RN license (permanent license required; temporary licenses not accepted) Minimum 2 years of inpatient case management experience (3 years preferred) Current BLS certification Knowledge of HMO and Medicare for inpatient, home health, and SNF settings Epic experience strongly preferred Proficiency with InterQual Criteria and Milliman Guidelines Understanding of discharge planning, care coordination, and plan of care development Two supervisor references from within the last year required at time of submission Why It's a Great Opportunity: This position offers the chance to make a meaningful impact on patient care through comprehensive case management in a well-supported clinical environment.
